Mary Marvel really is Billy Batson's sister, Mary Bromfield. Mary was adopted by the Bromfield family. Mary Marvel is part of the Marvel Family along with Captain Marvel, Captain Marvel Jr , Uncle Marvel, and Hoppy the Marvel Bunny.
